In preparation for his first trip to the United States on February 8, 1964, Lennon signs this medical card, listing his wife and dependents and directing that any death benefits should be assigned to his wife Cynthia who resides at 251 Menlove Avenue, Woolten, Liverpool. The recto could possibly be in Lennon's hand, but this is not conclusive. "Lennon / John / Winston / Date of birth Oct 9 1940 / Date signed: 15 Jan 1964 /". On the verso dependents are listed as Cynthia Lennon and erroneously as John Lennon Jr, instead of Julian. The document was later updated to list his mailing address as Apple Records, ATT Allan Steckler / 1700 Broadway, NYC 10019. On the recto at bottom, the control section shows the date of his death 12/8/80 and is red stamped below Lennon's signature in large letters "DECEASED." A very interesting document signed three weeks before Lennon's entry into the US to perform on "The Ed Sullivan Show."
